Changes made w/ replace text tool disappear

I'm using Acrobat Pro 10.1.6 on a MacBook Pro.

 

Sometimes -- not always -- when I make changes to a book-length PDF using the replace/insert text tool, those changes drop out, although the markers in the text remain. Deletions and highlights stick, but anything done with the replace/insert text tool is gone from the comments box.

 

Sometimes -- not always -- this happens after an Acrobat crash (which happens way more often than it should, and yes, I've maxed out my RAM).

 

A week ago, I sent a file to a client, and all of those replace/insert text changes had dropped out on their end, although they were fine when I sent it. When I went back to check and resend the file, they had all dropped out on my end, too. This file did not crash at any point.

 

Yesterday, I spent the day redoing the file and re-sent it to the client. Again, all of those replace/insert text changes have dropped out on their end, but they're fine here on my end.

 

This is incredibly frustrating and is costing me money. Anyone have any ideas what's going on here?
