Unable to create bookmarks in pdfs.

Hello,

I am unable to create bookmarks in the PDFs I have created. In the PDF I have created, I click on Documents > Add Bookmarks and only one untitled bookmark displays.

I have removed and installed Adobe Acrobat 3D Version 8 and Acrobat Distiller 8 numerous times, however, this has not solved the problem.

I have sent the Word file I used to create the PDF to other writers in my group and they are able to create a PDF with bookmarks.

Hopefully someone is familiar with this problem and has a solution.
